Your Options

All members of the OHSU community are encouraged to report sexual misconduct, intimate partner violence, or stalking that is perpetrated against them or other members of the OHSU community. There are numerous options for reporting the misconduct and obtaining support; which option a person chooses depends upon the nature and severity of the misconduct, whether the person wishes the report to remain confidential, and whether the person wishes to pursue a formal complaint.

To discuss these options and to make a non-confidential report of a possible violation of Title IX, report what you know to Laura Stadum, J.D., at either of the following:

The Title IX Coordinator can offer a variety of resolution options including, but not limited to: Informal resolution and mediation (as appropriate), training, providing support and referral to local/community resources, formal resolution and investigation.

Please use this Prohibited Discrimination and/or Harassment Complaint Form to fill out as much information as possible based on your knowledge of the situation. You may submit an anonymous report; however, without the contact information of the complainant and the ability to investigate further, the university may be limited in how it can respond. The following terms are used throughout this site and in the form:

  • Complainant: The person who submits the report. The complainant may be the victim, a witness or another person who knows about an incident or situation.
  • Victim/Survivor: The person who experienced the violation or harm.
  • Respondent: The person accused of perpetrating the violation or harm. If there are more than 2 respondents, please note that in the description of events.
  • Witness: a person who saw or heard an incident occur or who has relevant information about it.

What to Tell Us

Include in your report:

  • Known names of the involved parties.
  • The status of the parties (i.e., staff, student, faculty, guest, other).
  • The date of the incident.
  • The location of the incident, if known.

Provide as much information as you were given and do not press the person to provide more details or try to investigate the matter further. Do not wait for someone to complain if you know or suspect sexual misconduct is occurring or has occurred.

OHSU takes seriously all complaints of sex and gender discrimination and harassment, including sexual assault and sexual violence. Reporting a sexual assault can be difficult and OHSU is committed to respecting your privacy and limiting the sharing of your information to those people who have a need to know such information.

OHSU has an obligation to respond to sex discrimination and harassment complaints regardless of whether a person makes a police report and/ or whether a criminal investigation is proceeding. Because of these obligations and concern for the safety of the institution, OHSU and the Title IX Coordinator cannot guarantee complete confidentiality and may be required to investigate regardless of whether or not the individual reporting wishes to proceed.

Non-Retaliation Policy

Any attempt to retaliate or any actual retaliation in response to a Title IX report is strictly prohibited and may result in discipline up to and including discharge for employees and suspension or expulsion for students.

Retaliation includes: intimidation, threats, coercion, or adverse employment or educational actions taken against an individual because of the individual's participation in the reporting, investigation, or resolution of a complaint. Retaliation can be committed by: complainants, respondents, or any other individual or group of individuals. Attempted or actual acts of retaliation should be reported to the Title IX Coordinator.

Interim Measures

While an investigation or grievance proceeding is pending, OHSU will take interim measures as appropriate under the circumstances to assist or protect persons who were or may have been subjected to sexual misconduct, intimate partner violence, or stalking, as well as to protect the OHSU community from the misconduct reoccurring. Interim measures may include, for example, class schedule, or work schedule of either party. Interim measures may also include offering the Complainant and Respondent counseling, health care, academic support, or other resources.

During any stage of the investigation, if OHSU reasonably suspects that a Respondent poses a threat of harm or disruption to the campus community, OHSU may take immediate action.
