SOD Advanced Education Programs 2019-2020
The School of Dentistry offers six advanced education programs in General Practice Residency (GPR), Endodontology, Orthodontics, Oral and Maxillofacial Surgery, Pediatric Dentistry and Periodontology. Admission to a postgraduate program leading to a Certificate or combined Certificate/Masters is granted by the Advanced Education Committee. Applicants applying to our advanced education programs must be graduates of a CODA (Commission on Dental Accreditation) accredited dental school. Applicants with foreign dental degrees are not eligible for admission to our advanced education programs. Applicants to the OMFS and Pediatric Dentistry programs must be a US citizen or permanent resident. **GPR applicants who are not U.S. citizens or permanent residents must be legally able to work in the U.S. or eligible to obtain authorization to work in the U.S. Accepted applicants to our GPR, OMFS and Pediatric Residency programs must acquire a license to practice dentistry in the state of Oregon prior to matriculation. General application procedures
All applicants are required to submit an application through the Postdoctoral Application Support Service (PASS). PASS is a centralized application service for more than 460 participating postdoctoral dental education programs. All PASS application materials may be requested online at www.adea.org. Please follow PASS application instructions with regard to the submission of application materials such as transcripts and general letters of evaluation/recommendation. All programs require 3-4 PEF's (Professional Evaluation Form) be submitted through the PASS service. Supplemental materials such as a CV or undergraduate transcript may be uploaded to PASS. Please do not send these additional materials directly to our program unless you receive a specific request from Admissions personnel. Unsolicited material(s) will be shredded.

OHSU supplemental application and processing fee
OHSU no longer requires the Supplemental Application for Admission to Advanced Education Programs. However, a $75.00 non-refundable application processing fee must accompany your application. Application fees can be paid online.
Dental match
OHSU’s General Practice Residency, Oral and Maxillofacial Surgery, Orthodontic, Pediatric Dentistry and Periodontology programs participate in the Postdoctoral Dental Matching Program. If you are submitting an application to any of these programs you must register to obtain a Match number. Please include your Match number on your PASS application. To obtain a Match number please visit www.natmatch.com/dentres/.
Advanced Dental Admission Test (ADAT)
The Advanced Dental Admission Test (ADAT) is an advanced dental education admissions test designed to provide dental education programs with a means to assess program applicants' potential for success. The ADAT is not required for any Advanced Education program in the 2019-2020 application cycle. Graduate Record Exam (GRE)
Applicants applying to our Masters/Certificate Program in Orthodontics are required to take the General Test of the GRE. The test is composed of analytical writing, verbal, and quantitative sections. For general information or to request official score reports please visit www.gre.org. (OHSU’s GRE School Code: 4871, Department Code: 0604). Please do not send official score reports directly to our school during the application process. Official score reports are only required for matriculation.
Official Dental School transcripts and Professional Evaluation Forms (PEFs)
All application materials such as official dental school transcripts, national board scores and PEF's (Professional Evaluation Form) must be submitted to PASS. In addition to the mandatory institutional evaluation letter from the Dean of your dental institution, all programs require 3-4 PEF's be submitted through the PASS service.
- Endodontology: 4 PEFs required - we leave the source of these evaluations up to the applicant
- GPR: 3 PEFs required - we leave the source of these evaluations up to the applicant
- Oral and Maxillofacial Surgery: 3 PEFs required; 2 evaluations must come from oral and maxillofacial surgeons; we leave the source of the third evaluation up to the applicant
- Orthodontics: 4 PEFs required; 1 PEF must come from an Orthodontic instructor; we leave the source of the remaining 3 PEFs to the applicant
- Pediatric Dentistry: 3 PEFs required - we leave the source of these evaluations up to the applicant
- Periodontology: 3 PEFs required - we leave the source of these evaluations up to the applicant with the caveat that the recommenders can offer an evaluation of the applicant's clinical ability/performance
ADA National Board Dental Examinations Parts I and II
All students completing their dental education in the United States are required to complete the ADA National Boards Part I and II prior to graduation and licensure. All of our programs require ADA National Boards Part I scores, reported in PASS at the time of application. Accepted applicants must provide passing Part II scores in order to matriculate in our programs. Only ADA NBDE Part I & II scores will be considered. Please do not mail official NBDE results directly to our institution during the application process. Official NBDE results are only required for matriculation.
National Board of Medical Examiners, Comprehensive Basic Science Examination (NBME CBSE)
The NBME CBSE measures the understanding of the basic sciences and provides an enhanced mechanism for Oral & Maxillofacial Surgery (OMFS) training programs to evaluate applicants for residency positions. For the 2019-20 application cycle OHSU's OMFS program requires completion of the CBSE exam. CBSE exams taken after August 17, 2019 will not be accepted. CBSE scores should be dated within 2 years of the application deadline.
Graduate Medical Education (GME) programs
Graduate Medical Education (GME) training programs are offered in General Practice Residency, Oral & Maxillofacial Surgery and Pediatric Dentistry residencies. Residents are eligible for a stipend depending upon the level of postgraduate training the resident has completed in the United States. They also have funds from OHSU added to their pay to cover the cost of insurance coverage sponsored by the university. Financial aid
OHSU is committed to providing access to academic programs in an environment that stimulates the spirit of inquiry, initiative and cooperation and is similarly committed to helping student’s finance their education. Over 90% of students enrolled at the School of Dentistry receive some type of financial assistance. Both federal and private loans are available to US citizens and permanent residents of the United States. Financial aid applications are available in January/February each year on the OHSU Financial Aid website. Detailed information on financial aid can be obtained from the Office of Financial Aid site or (503) 494-7800.
*Note: Only Endodontic, Orthodontic and Periodontic students are eligible to receive financial aid.
Residency requirements
In Oregon, as in other states, instruction fees at publicly supported four-year colleges and universities are higher for nonresident students than for resident students. Currently, nonresident students are assessed instruction fees that approximate the full cost of instruction. The current policy used in determining residency seeks to ensure that only Oregon residents are assessed the resident fee.
Establishing residency
Your purpose for establishing residency in Oregon should not be primarily for educational reasons. Generally, you should reside and be employed in the state of Oregon at least one year prior to matriculation. If you are enrolled in classes in the state of Oregon, you should not be enrolled in more than eight (8) semester or quarter hours. The comprehensive document Student Residency (OHSU Policy 02-10-010) is available on the University Registrar's Office website. Any applicant in the process of establishing Oregon residency during the application cycle will not be considered an Oregon resident for the purpose of admission and/or tuition assessment until we have been able to confirm that Oregon residency requirements, according to OHSU's Residency Policy, have been satisfied.
Reapplication
Applicants who have submitted applications in previous years and are seeking to reapply to an advanced education program must submit an entirely new application and supporting documentation. No materials from previous applications will be used to complete your application documentation requirements.
Deferred admissions
OHSU does not offer an option to defer admissions. Accepted applicants who wish to postpone enrollment must reapply through PASS during a subsequent application cycle.
Matriculation and orientation
It is the responsibility of all admitted students to complete their admission files. A final set of official transcripts from all post-secondary schools (undergraduate and dental schools), official ADA National Dental Boards, Part’s I and II (if applicable) , and official GRE (orthodontic only) scores will be requested prior to matriculation. Not receiving this documentation in a timely manner will result in a hold being placed on registration privileges, financial aid, and transcripts. The Office of Admissions and Student Affairs will request this documentation prior to matriculation.
F-1 student visas
OHSU will provide an I-20 only to those students that gain admission to one of our advanced education programs. To acquire an I-20 you will be asked to provide a financial statement from a bank savings account that documents enough funds to cover the total cost of all educational and living expenses for one year.
Health requirements
All admitted students must meet OHSU's health and immunization requirements prior to matriculation. OHSU follows the guidelines for required immunizations issued by the Centers for Disease Control (CDC) and by the State of Oregon. Matriculating students in the Endodontic, Orthodontic and Periodontic programs will be asked to provide JBT Health & Wellness Center, OHSU's student health center, with proof of immunizations before Orientation. Students who are not in compliance 30 days after matriculation will have their registration privileges cancelled and be removed from school. Students must remain in compliance for the duration of their enrollment at OHSU. Criminal history reviews
Admitted applicants must complete a criminal offender inquiry and undergo a criminal history review as part of the matriculation process. Prior conviction does not necessarily disqualify an admitted applicant from matriculation. However, prior conviction may impede one's ability to obtain a license to practice dentistry. Applicants with a criminal history should consult with their state licensing agency for those instances where licensing may not be a possibility.
The School of Dentistry retains the right to rescind an accepted student based on any negative infraction as a result of our criminal history review. The admitted applicant has the right to inspect and challenge the results of the inquiry. Information about federal civil rights law and arrest records may be obtained through the Bureau of Labor and Industries.
Drug screening
All newly admitted students must successfully complete a drug screening within 14 days from the date of matriculation. This requirement is one way OHSU demonstrates its commitment to maintaining a safe and healthy environment for employees, students, patients and visitors. Positive test results may immediately disqualify an admitted student from enrollment. The admitted student has the right to inspect and challenge the results of the inquiry.
OHSU policy indicates that no person shall be under the influence of any unlawful drug, any alcohol, or any misused prescription drug while on OHSU premises. Students suspected of being under the influence in violation of this prohibition may be required to submit to a drug or alcohol test at cause.

OHSU employees as students
It is the responsibility of OHSU employees seeking to pursue individual courses for academic credit or academic degree or certificate within OHSU to notify the academic program of their employment status prior to registering or accepting admissions to ensure the activity does not impact their work schedule and to minimize potential conflicts of interest.
Current OHSU students intending to accept an employment offer (excluding positions identified as Student Employment or Student Work Study) from an OHSU department must notify their academic program prior to accepting the position.
Gainful employment regulations
Federal regulations require disclosure of certain information for graduate certificate programs that the U.S. Department of Education defines as "Gainful Employment " programs. Technical standards
The Oregon Health & Science University School of Dentistry is the only academic health center in the state and its fundamental purpose is to ensure and improve the health of all Oregonians. The ethical practice of dentistry requires intellectual ability, physical competence, and personal responsibility. Therefore, each applicant must satisfactorily complete all requirements for admission to the School of Dentistry. In addition to demonstrating personal characteristics appropriate for a career in a health care profession, qualification for admission to, and, following completion of the curriculum, graduation from the School of Dentistry requires satisfaction of the following standards:
- sufficient intellectual capacity to fulfill the curricular requirements of the various required basic science and clinical courses;
- ability to communicate effectively with patients, colleagues, faculty, staff, and other members of the health care community;
- ability to learn and safely perform the various technical skills required to complete the dental curriculum; and
- sufficient emotional stability and responsibility to withstand the stresses, uncertainties, and changing circumstances that characterize the practice of dentistry and dental specialties.
Specific technical standards, based on clinical competencies and accreditation standards of the Commission on Dental Accreditation of the American Dental Association, are available from the Office of Admissions and Student Affairs of the School of Dentistry.
