Gabriel Park is located in a community setting, single family and multi‑family dwellings, across from Gabriel Park - a pleasant retreat in a community setting. We are near the neighborhoods of Hillsdale and Multnomah Village, and close to Beaverton Hillsdale Hwy.
Time from OHSU: We are only 4.25 miles, a 10-minute drive from the campus of OHSU.

- Working with South West Community Health Center to develop and support a new safety-net clinic for the poor and vulnerable populations in the southwest community around our practice. This center opened May of 2005. Volunteer faculty and residents from all clinics as part of rotation responsibilities staff this new and much needed service.

Kaiser Permanente's Beaverton office is located in a suburban setting, among a diverse business district as well as single-family and multi-family dwellings, within two blocks of two parks. We are on Beaverton Hillsdale Highway, near 217.

OHSU Family Medicine at Richmond
Across the Willamette River from "The Hill" or main OHSU campus. It sits in southeast Portland, surrounded by an economically diverse neighborhood, lively business districts, and the expansive wooded parks of nearby Mt. Tabor.
Time from OHSU: 15 minutes by car across the Ross Island Bridge from OHSU to Richmond. Some residents bike to Richmond or live in nearby neighborhoods.
Physical Plant:
- Richmond was built from the ground up in the summer of 1995, and was designed as a community family medicine residency training center. Features include:

- 28 exam rooms –expanding to 30 exam rooms and five talking rooms by July 20156

Patient Demographics
Richmond patients reflect the economic diversity of the surrounding neighborhoods. Richmond became a Federally Qualified Health Center (FQHC) Look Alike in August 2004, and by June 2012 became a full grant-funded Community Health Center/FQHC (allowing it to care for a larger number of uninsured, Medicaid and Medicare patients from its service area which comprises a population of 75,000. The population is especially culturally diverse, from southeast Portland's Russian, Latino, Chinese and Southeast Asian immigrant populations. As is true of all OHSU family medicine practices, our patients range from the newborn to the elderly.
Clinical Resources
Richmond has tools that a community family physician would require in his or her practice, including sigmoidoscope, colposcope, tympanometer and very functional procedure rooms. Patient blood samples are sent by courier to the OHSU lab results are returned through OHSU's clinical computer network. Digital x-ray is on-site. Level three Patient Centered Primary Care Medical home which includes integrated behavioral health team.
Specialty clinics include ENT, cardiology, Pain Clinic, Orthopedics, Psychiatry, Podiatry, Sports Medicine, Integrated Movement Therapy and Acupuncture.

OHSU Family Medicine at Scappoose
OHSU Family Medicine at Scappoose is located about 25 miles northwest of Portland, in the medically underserved area of Columbia County, population 48,000. We provide routine, acute and preventive care for infants and children, adolescents, adults and the elderly. We offer women a full range of health care services, including annual exams, maternity and pregnancy care. We develop relationships with our patients that allow us to respond to their changing needs in every phase of their lives. We make a special effort to understand our patients' health histories, their family responsibilities and the things that are important in their lives. We respect their beliefs, values and cultural needs. We care for people —while we treat their illnesses or medical conditions.
Physical Plant:
- Opened in January 1998

- 30,000 patients per year, averaging about 2,421 patient visits per month.
