2018-2019 Student Health Insurance Plan (PacificSource)

OHSU requires all eligible students to carry major group health insurance. For the 18/19 academic year OHSU has partnered with PacificSource Health Plans to offer its University Sponsored Health Insurance Plan. Eligible students are automatically enrolled. Dependent coverage is not offered on the Student Health Insurance Plan. For students with other major group health coverage, an insurance waiver application will be available approximately one month prior to the term start date and must be completed before the due date each year to opt out of the Student Health Plan. Please note, there are separate waivers for both medical and dental insurance.

You can also log on to http://www.pacificsource.com/ohsu/ to view your plan summary, view your ID Card, or to get more information to better understand your plan.

Questions about opting out of the Student Health Plans? Please visit the insurance waiver application page to submit your waivers for the entire academic school year.  Any additional questions please contact USI, our health insurance broker, at 800-853-5899 or email them with your questions about waiving out of coverage, waiver criteria, or eligibility for the student health plan.
