OHSU offers live, interactive video conferencing to help coordinate transfer of patients from the hospital to a skilled nursing facility. 

Skilled nursing facilities are a growing part of the OHSU Telemedicine Network. Telemedicine enables an OHSU nurse to hand off a patient to a receiving nurse via video conferencing.

This allows receiving nurses to virtually meet patients who are being transferred to their facility and familiarize themselves with their medical history and specific needs. The result: fewer patient readmissions and greater patient/provider satisfaction. Patients are able to meet a member of their new care team and have their questions answered, which reduces patient anxiety.
