Billing and Insurance

How you pay for your healthcare depends on if you have health insurance and the type of insurance you have.


Some insurance companies require you to get authorization (approval) from them, a referral from your doctor or both before you get treatment. Be sure to ask your insurance company what they require before you get any treatment. Make sure you get any authorization or referrals you need. If you do not do this, you might have to pay all the charges, even if your insurance company normally pays for the type of treatment you receive.   


Please give us all your insurance information. This includes your main (primary) insurance information and any other insurance you have (secondary insurance). Always bring your insurance card to the hospital or clinic. If your insurance, address or phone number has changed since your last appointment, please let us know.  


Even if you have insurance, you may still be required to make a payment at each appointment or treatment. This your insurance co-payment. Please pay this when you come for your appointment.

Please contact your insurance company before your visit to OHSU to check your benefits (what the plan will pay for) and coverage (how much treatment or how many appointments). Be sure to ask your insurance company if they pay for treatment at OHSU.
