Make an Appointment in the OHSU Facial Pain Program

For an appointment in the OHSU Facial Pain Program, you need a referral from your primary care provider if you are a member of an HMO (for example, Kaiser Permanente) or your health insurance company if you have another health insurance plan. We do not accept self-referrals (no referral from a doctor or insurance company).

If you have had X-rays, MRIs or CT scans, please bring copies to your appointment. Please bring them with you, unless your doctor already sent them to us.

Before your appointment, please pre-register with OHSU. When you pre-register, OHSU collects certain basic information such as your name, address and insurance information. This helps us create your medical records, bill your insurance and avoid confusing your information with someone who has a similar name.

The OHSU Facial Pain Program is a national leader in the treatment of orofacial pain (mouth and facial pain) including trigeminal neuralgia (TN). Our program offers comprehensive pain care with a special emphasis on pain problems that are difficult to treat with surgery.

Referrals

Before you schedule your first appointment, our clinic must receive a completed Referral/Intake Form and recent relevant medical records from your primary healthcare provider and/or neurologist.

Our scheduling staff will contact you when your referral is processed. This referral is different from an insurance authorization (approval). If your insurance company requires an authorization, the provider's office referring you should get the authorization and give us the information.
