The OHSU Parkinson Center and Movement Disorders Program has the largest and most recognized movement disorders clinic in the Pacific Northwest. We provide care, consultation and expertise for neurological (nervous system) disorders that cause movement problems.
Many patients and their families rely on the center for individual care plans that promote health over illness. We also care for your emotional and social well-being.

Referrals
Before you schedule your first appointment, our clinic must receive a completed referral/intake form and recent relevant medical records from your primary healthcare provider, neurologist or both.
Have your doctor call our consultation line at 800-245-6478 to make a referral.
Our scheduling staff will contact you when your referral is processed. This referral is different from an insurance authorization (approval).
If your insurance company requires an authorization, the provider's office referring you should get that authorization and provide that information to us.
After your first appointment, you can schedule your own appointments by calling our scheduling line at 503-494-7772 (press Option 1).
Clinic visit
We believe in a team approach where you are the central player. Learn more about what your clinic visit will be like.
To prepare for your visit, please complete the required clinic forms below. Please mail them to our office at least two weeks before your appointment.
If your appointment is sooner than two weeks, you may either:
Fax the forms to Neurology Clinic, attention: Alma Kennedy at 503-418-9276 or bring the completed forms with you.
