Thank you for your interest in the OHSU Nerve Center.

To see a neurosurgeon at OHSU, you need a referral from your primary care provider if you are a member of an HMO (for example, Kaiser Permanente) or your health insurance company if you have another health insurance plan. We do not accept self-referrals (no referral from a doctor or insurance company).

Any patient a doctor refers is given priority scheduling. If your doctor refers you, or if you are a doctor referring a patient to OHSU, we schedule the first appointment as soon as possible. Our team approach ensures that new patients are seen within one week. We believe no one should wait for an appointment.

If you have had X-rays, MRIs or CT scans, please bring copies to your appointment. Please bring them with you, unless your doctor already sent them to us. 

Scheduling

Before you schedule your first appointment, our clinic must receive a completed Referral/Intake Form [link to Referral/Intake Form] and recent relevant medical records from your primary healthcare provider, neurologist or both.

Have your doctor call our consultation line to make a referral at 800 245-6478 (Outside Portland).

Our scheduling staff will contact you when your referral is processed. This referral is different from an insurance authorization (approval). If your insurance company requires an authorization, the provider’s office referring you should get the authorization and give us the information.

Follow-up appointments may be scheduled directly by calling our scheduling line at 503 494-7772. Choose option 1 when you are prompted.

New Patient Referral Clinic

The OHSU Nerve Center is conveniently located at the OHSU Center for Health & Healing in Portland’s South Waterfront district, at the foot of the Portland Aerial Tram.

Every Friday, our multidisciplinary team of specialists holds a combined clinic for new patient referrals. Anyone from our team may interview and examine your patient.  

Together, we review i maging studies and laboratory test results so that we can make the best multi-modality treatment plan for each patient.

At times, patients need additional testing or an internal referral before we can make treatment recommendations. This can often be done the same day to assist patients who travel long distances to receive medical care at OHSU.
