To refer a patient for medical or pediatric dermatology

Follow this process for any medical or pediatric dermatology condition (Includes referrals to the Center of Excellence for Psoriasis and Psoriatic Arthritis, patients with bullous diseases, patients with severe atopic dermatitis, and all other conditions normally seen in the medical dermatology clinic.):
 

Non-urgent Patient Referral (Medical or Pediatric Dermatology)

To refer a non-urgent patient to any of our Medical Dermatologists, please:

• Fill out this form and fax it and the patient's chart notes to 503-494-1228 (fax)
  
• Inform the patient that they will receive a phone call and/or letter in the mail with the information they will need to schedule an appointment. Alternately, the patient can call our main number 503-418-3376 to schedule an appointment.

Urgent Patient Referral (Medical or Pediatric Dermatology)

To refer an urgent patient to any of our Medical Dermatologists, please:

• Fill out this form and fax it and the patient's chart notes to 503-494-1228 (fax)
  
• Call our referring physician line at 503-494-5851 (voice) to schedule an appointment for your patient.
• (If you are referring a patient for surgery, please follow these instructions.)

Please note:

• If you are referring a severe atopic dermatitis patient to Dr. Jon Hanifin, and the patient lives more than 150 miles away from Portland, please be aware that the patient may need to spend two nights in Portland as Dr. Hanifin may require a follow-up appointment on the second day.
