General Information
Phlebotomy services will be provided to all registered outpatients for tests requested by an authorized health care provider. OHSU Registration services can be reached by calling 503 494-8505.
OHSU providers must ensure all orderable and miscellaneous tests are ordered in Epic Order Entry system as Lab Collect. Lab test names and synonyms along with complete test information can be found in the Test Directory. Timed test and glucose tolerance test patients must arrive with adequate time to check-in and complete collection(s) before clinic closure. Duplicate orders will be cancelled. Research orders are required to be placed into Epic Order Entry, with the exception of blinded studies, which will be processed from a manual requisition.
Urgent verbal orders will be accepted only from OHSU transplant coordinators on weekends or holidays. Coordinators are required to place verbal orders into Epic Order Entry no later than the next business day.
All non-OHSU referring providers must provide written or typed and signed orders. Orders can be faxed, mailed, or patient-delivered and have the following minimum information: Full Patient Name, Date of Birth, Full Provider Name/Address/Phone/Fax, Provider Signature, ICD-10 code(s) tests ordered, and any Special Collection or Transportation Requirements. Recurring orders can accepted for up to one year if clearly indicated on original written order.
Note: Outpatient phlebotomists perform peripheral blood draws on the arm or hand only. OHSU IV Therapy may be paged after peripheral collection has been attempted to perform blood collections draws from PICC or Port lines and all other sites. IV Therapy can take up to an hour to arrive.
