Yen Pham, RPh, BS, MBA
Chief Pharmacy Officer Pharmacy Services & Clinical Nutrition

Yen Pham is the Chief Pharmacy Officer of Pharmacy Services and Clinical Nutrition. Her responsibilities include oversight over all pharmacy and clinical nutrition operations including, finance, inpatient, outpatient, ambulatory, home infusion, and Pharmacy Benefit Management services. She is the co-chair of OHSU's Clinical Knowledge &Therapeutic Executive Committee (formerly known as Pharmacy &Therapeutic Committee) and has made significant progress standardizing the medical care patients receive at OHSU. Her professional interests include medication therapy optimization, business operations and finance, and efficient use of systems within the department of pharmacy and clinical nutrition.

David Bearden, Pharm.D.

Dave is the Assistant Director for Clinical Services with responsibility for system wide clinical and academic initiatives with a major focus on inpatient adult medical, surgical, and intensive care. He also serves as Professor and Chair in the Department of Pharmacy Practice with the Oregon State University/Oregon Health and Science University College of Pharmacy. His administrative interests include faculty and practitioner development, enhancement of interprofessional and collaborative patient care, and demonstrating the value of antimicrobial stewardship

Erin Corella, Pharm.D., BCPS, BCOP

Erin is the Assistant Director for Ambulatory Pharmacy Services (Oncology, Infusion, and Research Pharmacy). Erin has been a pharmacist at OHSU since 2002 and made the transition to administration in 2014. She serves as the Residency Program Director for the PGY2 Oncology Pharmacy Residency established in 2011 and acts as an ASHP guest surveyor for PGY2 Oncology programs. Erin's professional interests remain focused on creating the best patient experience for those with cancer who receive care within the OHSU system.

Michael Deam, RPh 

Mike is the Assistant Director Outpatient Pharmacy with responsibility for the daily operation of OHSU's seven outpatient sites. Mike brings his extensive community pharmacy experience to OHSU. Mike's professional interests include patient safety, pharmacy automation, workflow management, customer service, and pharmacy-based immunization programs.

Neil is the Assistant Director for Information Management and Medication Safety with responsibilities to inpatient, ambulatory, and outpatient pharmacy services. Neil oversees the medication use process within the Electronic Health Record and any associated technology. His interests include patient safety, workflow efficiency, project management, change management, and pharmacy residency training.

Joe Fazio RPh, MHA, MBI

Joe is the Assistant Director for Pharmacy Business Intelligence and Business Services. His responsibilities include reporting and analytics, finance and revenue cycle management, regulatory compliance and supply chain across inpatient, ambulatory and outpatient retail settings. Joe's professional interest centers around the use of business intelligence, data management principles and outcomes research in developing safe and efficient medication use systems.

Lam Nguyen, Pharm.D., MBA

Lam is Assistant Director for Inpatient Pharmacy Services. His Inpatient oversight encompass both the adult OHSU hospital, as well as, Doernbecher Children's hospital. His responsibilities include direct and in-direct oversight of inpatient clinical service lines, as well as, pharmacy distribution operations including USP 795/797/800 medication preparation and distribution, automation management, and perioperative satellite pharmacy operations. Lam's administrative practice and research interests include leveraging lean principles in the application towards continuous improvement in quality, safety, and financial stewardship. He has a passion for change management that elevates the pharmacists and technicians ability to practice at the top of their licenses. He is active in the Vizient Pharmacy Network and Chairs the Programming workgroup for the Programming, Innovation, and Engagement Committee. He also serves as Program Coordinator for the PGY1/PGY2 Health System Pharmacy Administration and Leadership w/ Masters Residency.

Amy Szczukowski, RPh.

Amy is the Assistant Director for Specialty Pharmacy with responsibilities for the URAC accredited Specialty Pharmacy Program, ambulatory and discharge services. She currently serves as a state legislative advocacy coordinator through a national pharmacy association. Amy's administrative interests include operational process improvement, transitions of care and managed care strategies.

Jeff Wassouf, Pharm.D.

Jeff is the Assistant Director of the of the OHSU Mail Order, Home Infusion and Compounding Pharmacy operations. While his professional background is primarily in Community Pharmacy, Jeff is interested in developing innovative Specialty and Home Delivery pharmacy service models. Other professional interests include Pharmacist-based immunization programs, Medication Therapy Management and Collaborative Drug Therapy Management programs and Professional Development programs for Student Pharmacists.

Jennifer is the Assistant Director Supply Chain and Compliance for pharmacy services encompassing inpatient, outpatient and ambulatory settings.  Her administrative focus is patient safety and service. This focus is driven by assuring adherence to industry regulations, OHSU policy and procedure and by maintaining a consistent and cost effective medication supply.

Andrew Albanese, Pharm.D., MBA

Andrew is the Operations Manager for the Ambulatory Pharmacy Services (Oncology, Infusion, and Research Pharmacy). He completed his PGY-1 Pharmacy Practice Residency at Peace Health Southwest and a PGY-1/PGY2 Health-System Pharmacy Administration Residency at Oregon Health and Science University. He also provides oversight of the OHSU Pharmacy Internship Program in collaboration with the inpatient operations manager. Andrew is passionate to serve patients through continuous process improvement, lean methodology, change management, automation utilization, and supporting the Oncology, Infusion, and Research teams.

Nancy Boyd, MBA

Nancy Boyd is the Pharmacy Finance Manager. Her responsible include pharmacy billing, departmental budget and financial analysis. Nancy performs analysis on a wide range of complex issues related to department funding, financial operations and administration. She is responsible for analyzing historical revenue and expense trends in support of the department's development of programmatic business plans for new services and staff expansion. The focus for the billing function is to ensure appropriate revenue capture and continually improving revenue cycle process. Her administrative interests include financial operations management, operational efficiency, data accuracy that lead to a clear understanding of the data and drive timely decision making, project management.

Sara Gunden

 Sara Gunden, Pharm.D., MBA
Sara is the Pediatric Clinical Operations Manager with oversight over the pediatric inpatient service lines including, general pediatrics, pediatric intensive care and neonatal intensive care. Her administrative interests include medication safety, enhancement of pediatric clinical service lines, pharmacist professional development and workflow efficiencies.

Susie Jiing, Pharm.D., BCOP

Susie Jiing is the Ambulatory Clinical Manager with oversight in the Oncology, Infusion, and Research Pharmacy Service lines. She completed her PGY-1 Pharmacy Practice Residency at OHSU and her PGY-2 Oncology Specialty Residency at UW Medicine. Susie practices as an oncology pharmacist and served as a clinical coordinator/supervisor before making the transition to administration in 2016. Susie is passionate about serving patients, supporting her team, and participating in the clinical expansion of Oncology and Research Services at OHSU.

Kristen has direct responsibility for the daily operations of the OHSU Home Infusion Pharmacy. Her administrative interests include patient safety, sterile compounding best practices, pharmacy operations technology, and transitions of care.

Jess Lassiter

Jessica Lassiter, Pharm.D., BCPS, CPPS

Jessica is the manager over Medication Safety, Quality, Drug Information/Drug Policy for Pharmacy Services. She works to improve patient safety throughout the health system through collaboration with pharmacy, nursing and provider colleagues. With the help of her team, she identifies opportunities for process improvement and leads safety and quality initiatives throughout the department and health system. In addition to medication safety and quality, Jessica manages the Drug Information and Drug Policy team. The drug information and drug policy pharmacists are not only responsible for researching and responding to medication information queries but also work to develop and implement institutional medication-use policies. Her interests include medication safety in special populations, process improvement and transitions of care.

Steve is the Front Office Manager for Pharmacy Services. His responsibilities include oversight of the day to day office operations team which includes HR processing, new employee onboarding, payroll time keeping, scheduling, office finances, room scheduling, travel, and other staff office and administrative needs.

Jessie Pavlinac, MS, RD, CSR, LD

Manager, Clinical Nutrition and Senior Instructor in the Graduate Programs in Human Nutrition. She received her BS in zoology from Oregon State University and her MS in nutrition science from University of Wisconsin, Madison and a Masters in Healthcare Management from OHSU. Jessie is an active member of the Academy of Nutrition &Dietetics and served as President in 2009-2010. For fun she and her husband, Randy, are charter members of the One More Time Around Again Marching Band (a 500 member all adult marching band) and members of the Oregon State University Alumni Band.

Lisa is the Specialty and Mail Order Pharmacy Operations Manager with responsibility over daily operations of the outpatient specialty and mail order pharmacy, medication assistance program, and the URAC accredited Specialty Pharmacy Program. Lisa strives to improve patient experience and access to specialty medications. Her professional interests include process improvement, patient safety, and project management.

Colleen is the Inpatient Clinical Adult Operations Manager with oversight over inpatient clinical service lines which include medicine, surgery, cardiology, solid organ transplant, intensive care units and the emergency room. Her administrative interests include patient safety/experience, enhancement of clinical services, evidence based practice guidelines and continued interdisciplinary care, workflow efficiencies, transitions of care, and staff development.

Eric oversees support for the medication use process and information technology infrastructure at OHSU. His professional interests include innovative use of technology to improve patient safety and experience, creating workflow efficiencies and aligning healthcare needs with technological developments.

Patti is an Inpatient Operations Manager with responsibility over central pharmacy services with an emphasis in sterile and non-sterile compounding, controlled substance management, pharmacy automation, and medication distribution. She is responsible for managing inpatient technician workflows, overseeing the Technician Validation and Decentralized programs. Her administrative interests include automation and technology, medication safety, lean process improvement, teaching and certification activities.

Long D. Trinh is an Inpatient Operations Manager, Pharmacy Services with responsibilities for adult and pediatric operations, including automation and technology, sterile and non-sterile preparations. Dr. Trinh also provides oversight of the Intern Program in collaboration with the ambulatory operations manager. He currently serves as an advisor for the American Society of Health-System Pharmacists (ASHP) Pharmacy Forecast Student Workshop and on the Board of Directors for the Oregon Society of Health-System Pharmacists (OSHP);and served on the ASHP Innovation Management Section Advisory Group. His professional interest includes patient safety, operational efficiency, leadership development, financial and change management. His long term goals are to enhance the access and transition of patient care through innovative business models, interdisciplinary collaborations, and mentorship.

 

Nikki Whaling

Nikki Whaling, Pharm.D.

Nikki Whaling is the Operations Manager for Pharmacy Benefit Management Services. Nikki supports implementation of services and manages daily operations. Her professional interests include operational efficiency and leveraging data to improve outcomes.

Karey Whitten is the Nurse Manager of Home Infusion Pharmacy Services. Her responsibilities include oversight over all home infusion pharmacy nursing operations. Karey closely collaborates with the inpatient and ambulatory nursing leadership teams to support OHSU Nursing's Magnet status. Her professional interests include optimizing home infusion therapy practices, advancing nursing professional practice and research, nursing education, and increasing collaboration in practice and processes within nursing and pharmacy services.

Krista Wolf

Krista Wolf, Pharm.D.

Krista Wolf is the Manager of Research Pharmacy Services. She oversees the use of medications utilized in research at OHSU and ensures that the research pharmacy follows regulatory, institutional, and study specific requirements. She closely collaborates with Oregon Clinical and Translational Research, the Knight Clinical Research, and Pharmacy Management to ensure the highest standards are maintained for the management of research medications.

Jason's administrative practice and research interests include deconstructing the sources of stress and reward that maximize resilience and decrease burnout in a complex healthcare environment. He has a passion for health system operations, safety, financial stewardship, and people development. He is active within ASHP and serves as faculty for the Clinical Skills Certificate for International Pharmacy.
